过敏性紫癜患儿血清低糖基化IgA1、核因子κB与炎症递质的研究

Research in the relationship of inflammatory mediators with serum Gd-IgA1 and NF-κB in children with Henoch-Schönlein purpura

摘要: 目的 探讨儿童过敏性紫癜(HSP)血清低糖基化IgA1(Gd-IgA1)、核因子-κB(NF-κB)水平变化,及其与炎症递质间的关系。方法 选取2014 年10 月至2015 年3 月初发HSP患儿70 例,分普通组(23 例)、腹型组(21 例)、肾炎组(26 例);同期17 例健康体检儿童为正常对照组。采用ELISA法检测血清Gd-IgA1、NF-κB、白介素8(IL-8)、肿瘤坏死因子α(TNF-α)、细胞间黏附分子1(ICAM-1)水平。结果 在HSP急性期,普通组、腹型组、肾炎组及对照组间血清Gd-IgA1、NF-κB、IL-8、TNF-α、ICAM-1水平的差异有统计学意义(P均 < 0.01);其中肾炎组的血清Gd-IgA1、NF-κB、IL-8、TNF-α、ICAM-1 水平均高于普通组,差异有统计学意义(P 均 < 0.05)。在HSP恢复期,肾炎组的血清Gd-IgA1、NF-κB、TNF-α、IL-8 水平仍高于对照组,差异均有统计学意义(P均 < 0.05),ICAM-1水平在四组间的差异无统计学意义(P > 0.05)。HSP患儿急性期血清Gd-IgA1与NF-κB呈正相关(r = 0.67,P < 0.05)。结论 Gd-IgA1可能通过激活细胞NF-κB,上调炎症递质IL-8、TNF-α及ICAM-1的表达,参与介导 HSP的小血管炎性损伤。

Abstract: Objective To investigate the changes of serum levels of galactose-de?cient IgA1 (Gd-IgA1) and nuclear factor-kappa B ((NF-κB) in children with Henoch-Schönlein purpura (HSP), and the relationships of them with inflammatory mediators. Methods A total of 70 hospitalized children with HSP were recruited from October 2014 to March 2015, and divided into general group (23 cases), gastrointestinal group (21 cases) and nephritis group (26 cases). Meanwhile, 17 healthy children were selected as control group. The serum levels of Gd-IgA1, NF-κB, IL-8, TNF-α, and ICAM-1 were detected by ELISA method. Results In the acute phase of HSP, the serum levels of Gd-IgA1, NF-κB, IL-8, TNF-α, and ICAM-1 were significantly  different among general group, gastrointestinal group, nephritis group and control group (P all < 0.01). The serum levels of Gd-IgA1, NF-κB, IL-8, TNF-α, and ICAM-1 were significantly higher in nephritis group than those in control group (P all < 0.05). In the recovery phase of HSP, the serum levels of Gd-IgA1, NF-κB, TNF-α, and IL-8 in nephritis group were still higher than those in control group (P all < 0.05), but the level of ICAM-1 was no differenct among four groups (P > 0.05). There was a positively correlation between Gd-IgA1 and NF-κB in acute phase in children with HSP (r = 0.67, P < 0.05). Conclusions Gd-IgA1 may up-regulate IL-8, TNF-α, and ICAM-1 by activating NF-κB, resulting in small vascular inflammatory injury of HSP.
